BSA ANNUAL MEDICAL FORM
The BSA requires that all members, youth and adults, have an annual medical evaluation. The forms have four parts. Part A is a medical history, Part B is a legal release, Part C is a physical that must be done by a doctor, and Part D is a detailed information sheet for high adventure - like our summer trips for senior Scouts.
Troop 281 requires that Parts A, B and C be done for everyone annually. It is best to address Part D at the same time if you plan on going on one of our high adventure trips.
The form is made as a PDF that you can type into on your computer and then print out to take to the doctor. We encourage doing this to make sure that the information is legible if an emergency should arise.
Once completed, the signed forms are given to our Assistant Scoutmaster - Medical. They are taken on our outings and secured by a responsible adult leader. Information is shared only as needed with the adult leaders on each outing.
