Medical Form
BSA Scouts and Troop 281 requires that all members, youth or adult, provide health information and have an annual medical evaluation.
The medical forms have four parts, A, B and C must be done by everyone annually and part D is used only for High Adventure events:
- Part A – medical history
- Part B – legal release
- Part C – physical that must be done by a doctor
- Part D – HIGH ADVENTURE ONLY – Philmont, Seabase, etc…
We recommend completing the PDF form on your computer and then printing it for your doctor to complete. This makes the information more legible, should an emergency should arise.
Once signed and completed, give the forms to our Assistant Scoutmaster – Medical. Forms are taken on all outings and secured by a responsible adult leader. Medical information is reviewed and utilized only as needed.
