Campout Medication Form

If your Scout takes medication – prescribed or OTC – you must complete the Campout Medication Form for EACH campout.

• An adult leader will be responsible for administering the medication on the outing.  Scouts may not self-administer medications on outings.
• Please send the medication in its original packaging – it is not acceptable to send a baggie of pills.
